Jackson Transition Information

ARE YOU READY to transition to Jackson!
During your P1 year, you received your UMMC email address and UMMC student number. You will need both of these moving forward. If you do not recall this information, you will need to go back to the original email to retrieve it. If your password has expired or you have forgotten it, you can go to Password Central to reset it, or you can call the UMMC Help Desk to assist you with resetting it. The UMMC Help Desk can be reached at 601-984-1145.
Orientation:
August: Participation in Orientation is mandatory for all P3 students.
Submitting Documents:
There are two different places that you will need to submit the required documents. Some documents will go to both places, and some will go to only one place. Instructions are provided in each section below. The first place is REDCap. REDCap is provided and monitored by UMMC. You will receive an email from Studenthealth@umc.edu the week of April 1, 2024, that will provide a link to a REDCap survey where you will upload the completed documents as instructed below. Included in this packet is a separate document with more detailed instructions regarding REDCap. The other location is E-Value, which you have used previously. Make sure you carefully read the instructions regarding submitting/uploading documents.
The Student Health Services Registration Form provided must be completed and uploaded to REDCap by May.
Documentation of Immunizations:
As a P3 student on the UMMC campus, you will be required to follow the UMMC Immunization Policy. A copy of the UMMC Student Immunization Policy is included in this information packet. Your required immunization records must be on file in the UMMC Student Employee Health Center. Immunization records are official documentation showing that you received the required immunization, the location in which you received it, and the date it was given. Immunization information will need to be provided on the UMMC Immunization Records Form completed/signed by a licensed healthcare provider (physician, NP, nurse, PA, pharmacist, or public health official) OR a Form 121 (Certificate of Immunization Compliance) obtained from the State Department of Health in the state you resided when received. The UMMC Immunization Record Form has been provided to you. The completed/signed form will need to be uploaded to both REDCap and E-Value by May 6. All required immunizations must be shown on the official form you submit.
Note:The UMMC Immunization Policy was updated in 2021 to reflect a change in the Hepatitis B requirement and in 2023 to reflect a change in the COVID-19 vaccination (SARS-CoV-2). It is important that you read the UMMC Immunization Policy carefully and thoroughly to ensure you have met all requirements. Please note the requirements regarding the hepatitis B surface antibody titer.
Tdap:
If your Tdap expires before May 2026, you must have a new Tdap administered now. You cannot wait until it expires. If your current Tdap does not expire before May 2026, then you are good to submit your current one. You need to upload official documentation of your Tdap to both REDCap and E-Value by May 6.
Documentation of TB Skin Test:
As a P3 student on the UMMC campus, you will be required to follow the UMMC Student TB Screening Policy. A copy of the UMMC Student TB Screening Policy is included in this information packet. You are required to have a documented TB skin test within three (3) months prior to starting classes on the UMMC campus. The three (3) month period prior to starting classes begins May 15, 2024. The deadline to have your TB skin test completed (administered and read) is July 14, 2024.
You must have an initial two-step TB skin test with an annual (meaning within a 12-month period) TB skin test each year following.
Note:
A two-step TB skin test is having the test administered and read two days later, then repeating this process (administered and read two days later) a second time within 28 days of the first one.
- If you never received the initial two-step TB skin test, then you will be required to have a two-step this year.
- If you had the initial two-step TB skin test, but your annual TB skin test following was outside of the 12-month period, OR you did not receive an annual TB skin test, then you will need to start over with a two-step this year.
- If you had the initial two-step TB skin test AND received your annual TB skin test(s) consistently within the 12-month period, then you will receive the single-dose TB skin test this year as long as your 12-month period falls between the May 15, 2024 – July 14, 2024 period.
You must upload your TB skin tests from the previous two years (P1 and P2 year) to both REDCap and EValue by July 15.
- TB skin testing is provided at no cost at UMMC Student Employee Health (Jackson) before July 12 on Monday, Tuesday, Wednesday, or Friday (not available on Thursdays) between the hours of 7:00 a.m. - 4:00 p.m. You will need to request documentation from UMMC in order to upload the documentation of your TB skin test to both REDCap and E-Value by July 15.
- If you have your TB skin test administered somewhere other than UMMC, you must upload official documentation from the clinic where the TB skin test was done to both REDCap and E-Value by July 15.
- If you have had a positive TB skin test in the past, refer to the UMMC Student TB Screening Policy for further instructions. You may contact UMMC Student Employee Health at 601-984-1185 or studenthealth@umc.edu for further assistance.
All students are required to have a physical exam prior to starting the P3 year. This exam can be done at the University Student Health Center (Oxford) or by a healthcare provider of your choice. UMMC Student Employee Health does not provide physical exams. A physical exam verification form has been provided to you today for your provider to sign upon completion of your physical exam. Make sure a current copy is uploaded to E-Value by July 15. This form is not to be uploaded into REDCap.
If you previously requested a vaccine accommodation/exemption, please make sure the official approval documentation is uploaded to REDCap. If a vaccine accommodation/exemption needs to be made for a required vaccine, you will need to submit that request through the UMMC Student and Employee Health Accommodation/Exemption Request site. You will need to notify Ms. Sheri Conner (swconner@umc.edu) that an accommodation/exemption request is being made. Once you receive the decision from UMMC, you will need to upload that document to both REDCap and E-Value. You will also need to email a copy to Ms. Conner.
You are required to have a background check administered by UMMC Human Resources (HR) on the Jackson campus or Grenada campus before July 10. Information on scheduling this appointment is included in this information packet. Please schedule your appointment as soon as possible to ensure availability. When you go to HR for fingerprinting, you must take a valid driver’s license (from any state), a valid passport, or a valid state ID card (from any state). (A Mississippi State ID Card can be obtained at the Mississippi Highway Patrol’s office.) If you do not bring one of these forms of identification, you will NOT be able to be fingerprinted for your background check. Once all needed information is received and verified, you will receive a UMMC Clearance Letter by email. Please upload a copy to E-Value to the “Background Clearance” section by July 15.
On the same day that you have your background check, you will need to have your photo taken for a UMMC ID badge. Human Resources (on the Jackson or Grenada campus) will take your ID picture after you have been fingerprinted for the background check. Make sure that you let the HR office know you need your ID taken before you leave their office. Again, you must have a valid driver’s license, passport, or state ID card with you to get this photo taken. You will receive your ID badge at the P3 School of Pharmacy Orientation in August. This photo must be taken before July 10 and should be done at the same time/place as fingerprinting.
Note:
Your fingerprinting and ID photo must be administered by UMMC HR on the Jackson campus or Grenada campus, so you need to take care of this before you leave to go home for the summer if you live further away from either of these locations. If you fail to do so, you will have to drive back to one of these locations before the deadline. No exceptions.
You are required by the School to maintain professional liability insurance. Upload current documentation of your liability insurance to E-Value by July 15. This documentation is not to be uploaded into REDCap.

Basic Life Support for the Health Care Provider (BLSHCP) training is required for all students. A BLSHCP course trains students to perform adult, child, and infant cardiopulmonary resuscitation (CPR); manage foreign body airway obstruction in the adult, child, and infant; and defibrillate utilizing an Automated External Defibrillator. This certification will be needed for all of your rotation experiences, expires every two (2) years, and must be renewed. Students are required to complete basic life support (BLS) certification before beginning P1 classes and then again prior to P3 classes. This course will need to be valid through April of 2026 in order to participate in all P3 and P4 rotations. You will need to complete a BLS instructor-led training program through an approved American Heart Association program. Another option is to complete an online/hybrid course through UMMC using this link: https://ummc.enrollware.com/schedule. If you choose this training, select the “BLS (online/hybrid via RQI)” option; choose your preferred date, and the “UMMC Student/Employee Self-Pay” option for $45.
You will need to upload your completed BLSHCP Certification (AHA electronic certification card that will be emailed to you) to E-Value by July 15. This certification is not to be uploaded into REDCap. Please contact Dr. Laurie Fleming (lwfleming@umc.edu) if you are unsure about a course prior to a purchase. A guide to finding a BLS course will be placed on the Class of 2026 Blackboard.
UMMC requires that all students receive an annual flu vaccine.

REDCap is the web application provided and monitored by UMMC to manage online data entry, surveys, and databases. REDCap is where you will submit UMMC student health registration and required documents.
You will receive an email from Studenthealth@umc.edu with a link to register for REDCap. The email you receive is personalized to each student and is not to be shared. It is very important that you save this email. Once you click on the link, you will complete the requested information. Make sure that you provide your first and last name, your birth date, and UMMC email address so that you can easily come back to the site/form later. This application does not need to be completed at one time, but in order to return, you must complete the required fields listed above. Please note that extra spaces should not be added after your name and email address, or you may have trouble logging back in. REDCap is case and space-sensitive.
UMMC Student ID #: You received this at the start of your P1 year and should enter that in the form.
Please upload your UMMC Student Health Packet: This is the UMMC Student Health Services Registration Form (titled Part I) and the Immunization Records Form (titled Part II). (A completed/signed Form 121 will be accepted in place of the UMMC Immunization Form if it contains all required immunizations.) A physician/nurse must complete your Immunization Records Form or Form 121. All physician/nurse information must be completed on the form. It is your responsibility to ensure location details, vaccinations, and bloodwork (if applicable) are answered. UMMC Student Health will not accept any vaccinations or bloodwork documented on the form after the form has been signed.

Complete: Once you have uploaded the required documents, you will need to select 'complete' here.
Must provide value: This means that it is a required field.
If you close out of REDCap and wish to return at a later time to submit documents, you will need to go back to the REDCap email and click on your personalized link. Once you click the link, a page will pop up asking for your information (name, date of birth). You will then hit 'log in,' and it should take you back to where you upload submissions. You do NOT need a username/password.
